The graphic physicality of this curse is not gratuitous but theologically precise. In Hebrew thought, the *me'im* (מֵעַיִם, 'bowels / intestines') were the seat of deep emotion, compassion, and the felt inner life — the organ of the self that loves and grieves most deeply (cf. Gen. 43:30; 1 Kings 3:26). To have them rot and fall out is the most visceral possible judgment on a man whose inward corruption has led an entire nation astray. The punishment mirrors the crime: the rot within made grotesquely and inexorably visible.
