Τετύφωται (perfect passive of τυφόω) carries two conjoined images: to be "wrapped in smoke" (blinded by self-delusion) and to be "puffed up" (inflated by arrogance). The F.O.G renders "smoke-blinded" to foreground the perception-failure; the F.O.G Message renders "swollen with conceit" to foreground the pride dimension. Meanwhile νοσῶν ("being sick/diseased") directly echoes v.3's ὑγιαίνουσιν — the false teacher is precisely the patient who refuses the physician's wholesome medicine.
